Care Plan Template
[Back to Top]- The Global Care Plan Template Library administrative role’s functionalities have been moved to the following Agency Wide and Administrative Roles for Care Plan module:
- Care Plan Template Submit
- Care Plan Template Approve
- Care Plan Template Delete
- Care Plan Template View
These roles will be enabled by default for active users who currently have the Global Care Plan Template Library administrative role enabled. - Users will be able to take the following actions based on the roles:
- Care Plan Template Submit: View Care Plan Template in New, In Prep, Pending Approval, Returned, and Approved statuses; Save and Submit New, In Prep, and Returned Care Plan Templates; Copy Care Plan Templates in Approved status; View Update History.
- Care Plan Template Approve: View Care Plan Template in New, In Prep, Pending Approval, Returned, and Approved statuses; Save and Approve Care Plan Templates in New, In Prep, Pending Approval and Returned statuses; Return to Submitter in Pending Approval status; Copy an Approved Care Plan Template; Submit Approved Care Plan Template for global approval; Import Care Plan Template from Global Care Plan Template Library; View Update History.
- Care Plan Template Delete: Delete Care Plan Templates in In Prep, Pending Approval, and Returned status; Discontinue Approved Care Plan Templates; View Deleted and Discontinued Care Plan Templates.
- Care Plan Template View: View Approved Care Plan Templates.
- Individual Care Plan Submit + Any Care Plan Template role: Apply Approved Care Plan Templates to Individuals.
- From this release onward, users will also be able to approve Care Plan Templates and apply to individuals in their caseloads without having to send it to Therap Admins for global approval. The templates that have been submitted for global approval prior to this release will be approved by default for the respective provider.
- A new List link has been added beside the Care Plan Template option in the ‘Care Plan’ section of the Health tab. Clicking on it will open the ‘Care Plan Template List’ page containing the approved Care Plan Templates.
- Users assigned with Care Plan Template Submit role will be able to save and submit Care Plan Templates. Users assigned with the Care Plan Template Approve role will be able to save and approve Care Plan Templates.
- Saving or submitting a Care Plan Template will keep the form in ‘In Prep’ or ‘Pending Approval’ status, respectively. Users will be able to:
- Save the form by clicking on the Save button.
- Submit the form in ‘In Prep’ status by clicking on the Submit button.
- Return the form in ‘Pending Approval’ status only by clicking on the Return to Submitter button. The form will then be in ‘Returned’ status.
- Approve the form by clicking on the Approve button.
- Delete the form by clicking on the Delete button.
- Once a Care Plan Template is approved, users will be able to:
- Apply the template to an individual in their caseload by clicking on the Apply to Individual button.
- Submit it for global approval by clicking on the Submit to Global Template Library button. This button will not be available in Approved forms if the template has been submitted for global approval.
- Copy the template by clicking on the Copy button. The copied template will be in ‘New’ status. Users need to save, submit, or approve the copied form.
- Discontinue the template by clicking on the Discontinue button. Once discontinued, the template cannot be used.
- Clicking on the Apply to Individual button will take users to the ‘Select Individual’ page. Selecting an individual will open a new Individual Care Plan form for the individual with the applied template.
- Clicking on the Submit to Global Template Library button will prompt a pop-up window asking if the user would like to proceed. Clicking Yes will take them to the ‘Global Care Plan Template Library User Agreement’ page, where users can click on the Continue button to submit the template to the Global Care Plan Template Library after selecting the suitable option.
- Form ID, Returned By, Approved By, and Discontinued By fields and an Update History link have been added to the popup accessible from the Care Plan Template form’s ‘i’ icon. The Update History link will be available to the users who have the Care Plan Template Submit or Care Plan Template Approve administrative role enabled. Clicking on the Update History link will open the ‘Update History’ pop-up window.
The Submitted By field that appeared in the popup once a template is submitted for global approval has been renamed to Globally Submitted By.
If the template is imported from Global Care Plan Template Library, then the Global Template Form ID field will be available in the popup. - A new field titled Global Template Form ID has been added to the SComm message generated by the system when a Care Plan Template has been globally published or discontinued.
- The user interface and the workflow for the published Global Care Plan Template has been updated. The imported template will be in ‘In Prep’ status and users will need to approve the template to apply it to an individual. The following additional changes have been made to the template:
- A new ‘i’ icon has been added. The Author and Published By fields have been renamed to Entered By and Globally Submitted By, respectively.
- The Rating and My Rating fields have been removed from the ‘Popularity’ section.
- The ‘Keywords’ section has been removed.
- The Save button has been removed. Users will need to directly import the template.
- The following changes have been made on the success message page displayed after importing a Global Care Plan Template:
- A Form link has been added which will open the Global Care Plan Template form.
- A List link has been added which will open the ‘Global Care Plan Template Library’ search result page.
- An Open Imported Care Plan Template link has been added which will open the imported template in ‘In Prep’ status.
- A Create New Care Plan Template link has been added which will open a new Care Plan Template form.
- The PDF link has been removed.
- Users will receive a pop-up window for confirmation if they try to import an already imported template.
- The Records dropdown field has been removed from the ‘Approach(es)' section in Care Plan Template and Global Care Plan Template forms.
- Users will receive the following error message if they try to create or approve a template with the same name as an existing one:
“This Care Plan Template Name has already been used” - The following changes have been made in the ‘Global Care Plan Template Library’ search page:
- The Template Name/Keywords field has been renamed to Template Name.
- The Publish Date From field has been renamed to Globally Submitted Date From.
- The Publish Date To field has been renamed to Globally Submitted Date To.
- The Author First Name field has been renamed to Entered By First Name.
- The Author Last Name field has been renamed to Entered By Last Name.
- The Author Provider field has been renamed to Provider.
- The following changes have been made in the ‘Global Care Plan Template Library’ search result page:
- The Keywords and Rating columns have been removed.
- The Publish Date column has been renamed to Globally Submitted Date.
- The Author column has been renamed to Entered By.
- The Author Provider column has been renamed to Provider.
- The result will be sorted by the Import Count column.
- Two new fields titled Form ID and Entered By have been added to the ‘Care Plan Template Search’ page.
- A new column titled Form ID has been added to the ‘Care Plan Template Worklist’ and ‘Care Plan Template Search’ result pages.
- The following changes have been made in the Activity Tracking module:
- The 'Template Create/Template Update', 'Template View', 'Template Submit', and 'Template Delete' actions have been renamed to 'Save', 'View', 'Submit to Global Template Library', and 'Delete', respectively.
- New actions titled 'Submit', 'Return', 'Approve', and 'Discontinue' have been added.
- Care Plan Template Form IDs will be displayed in the Form ID column.
- Comments have been removed except for the 'PDF View' activity type.
Individual Care Plan
[Back to Top]- The caseload-based roles for Care Plan module have been renamed as following:
- Individual Care Plan Submit
- Individual Care Plan Approve
- Individual Care Plan View
- Individual Care Plan Delete
- Individual Care Plan Review
- Individual Care Plan Update
Active users assigned with the Individual Care Plan Submit caseload-based role will have the Care Plan Template View administrative role enabled for them by default. - The ‘Individual Care Plan’ section has been renamed to ‘Individual Care Plan Details’ section.
- Users will now require the Individual Care Plan Delete caseload-based role to delete an Individual Care Plan and can no longer delete an Individual Care Plan with the Individual Care Plan Submit or Individual Care Plan Approve caseload-based roles.
- The Records dropdown fields have been removed from the ‘Approach(es)’, ‘Reviews’, and ‘Team Meeting Discussions’ sections.
- A new Template Form ID field will be available in the popup accessible by clicking on the ‘i’ icon if the Individual Care Plan form is created using a Care Plan Template. Also, the Time Zone field will now show the Time Zone of the individual instead of the Provider.
- A new ‘i’ icon has been added to the Review page.
- An ‘i’ icon has been added next to the Individual field in the Individual Care Plan, Reviews, and Team Meeting Discussion pages, which will show the links to the Individual Demographic Form (IDF) and Individual Home Page to the users having appropriate roles.
- The Time Zone column has been removed from the Pending Acknowledgement Report.
- A new Template Form ID field has been added to the exported PDF of the Acknowledgement Report and Pending Acknowledgement Report for Individual Care Plans created using a Care Plan Template.
- Clicking on the Display PDF button in available pages of Individual Care Plan will now open the PDF in the PDF viewer. The downloaded PDF name has been changed from ‘Care Plan’ to ‘Individual Care Plan’.
- The following changes have been made on the success message page after saving a new Individual Care Plan:
- A Create New Individual Care Plan link has been added which will open a new Individual Care Plan for the selected individual.
- The List link has been removed.
- If the form is created using a Care Plan Template, then two additional links titled Apply the Template to another Individual and Back to Approved Template List will be available.
- The Import field has been removed from the ‘Notification Events’ section of the Custom Notification Profile for the Individual Care Plan module. Users will no longer receive notifications for importing Care Plan Templates from the Global Care Plan Template Library.
- The following changes have been made in the Activity Tracking module:
- Comments have been removed for all actions of the Individual Care Plan module except for the 'PDF View' activity type in Activity Tracking module.
- The 'Import' action has been removed.
Case Note
[Back to Top]- A new ‘i’ icon has been added beside the Billing Unit(s) field of the Case Note forms for which Billing Unit(s) is marked as visible in the Case Note Template. Clicking on this ‘i’ icon will display the following Note: Billing Unit(s) will be used for billing if Calculation of Units is set as "Use direct billing units".
Comprehensive Health Assessment Tool (eCHAT)
[Back to Top]- On the ‘Skin and Wound’ page accessed using the Edit button in the ‘Skin and Wound’ section of the ‘Comprehensive Health Assessment’ (eCHAT) form, a new option titled ‘No skin integrity concerns at this time’ has been added for the Skin integrity: Individual is field. This new option has no effect on the acuity. Therefore, selecting this option for the Skin integrity: Individual is field will not result in the Summary Report’s HCP column displaying ‘R’ (denoting ‘Required’) for the ‘27 Skin and Wound’ and the ‘27.a (Skin integrity)’ rows.
Daily MAR
[Back to Top]- If users assigned with the Medication History Count caseload-based role attempt to navigate away from the Daily MAR that contains at least one active medication with Drug Count Enabled, then they will receive a warning message. If there are no Drug Count Enabled medications on the Daily MAR or the user does not have the role assigned, then the current warning message for navigating away from the Daily MAR will be shown.
Drug Count
[Back to Top]- The following features will now be applicable for the users who have the Medication History Count caseload-based role assigned:
- The ‘Medication History List’ that appears while creating a new Drug Count record from the Health tab will now show only the medications that are ‘Active’ for the current date. Medications that have already been discontinued or passed the End Date will not appear on this list. However, Drug Count for the inactive medications can be added from the corresponding Medication History and MAR forms.
- Drug Count has been added as a new ‘Notification’ module in the Module Name dropdown field of the Notification Profile Configuration. Users will now be able to receive notifications when there is a mismatch between the Amount on Hand and Amount Left values of the recorded Drug Count and the Drug Count recorded before that.
- The ‘Medication History List’ that appears while creating a new Drug Count record from the Health tab will now show only the medications that are ‘Active’ for the current date. Medications that have already been discontinued or passed the End Date will not appear on this list. However, Drug Count for the inactive medications can be added from the corresponding Medication History and MAR forms.
Event Summary

[Back to Top]- For agencies where the Order module is enabled, a new item titled ‘Active Orders’ will now be available for PDF configurations on the ‘IDF Provider Configuration’ page. The PDF files generated from the ‘View PDFs’ section of an individual’s Individual Demographic Form (IDF) or Individual Home Page will list the individual’s ‘Active Orders’ if the associated PDF configuration has the item ‘Active Orders’ selected for it.

[Back to Top]- A new Drug Count option containing the New and Search links has been added in the ‘Modules’ section on the Home tab of the Individual Home Page. Users assigned with the Medication History Count caseload-based role will be able to record or search for Drug Counts from here.
Intake/Elimination
[Back to Top]- Sub-options within the Column(s) list of the Intake/Elimination Configuration pages can now be selected or deselected independently. Selecting or deselecting the sub-option(s) will automatically add or remove the corresponding Intake/Elimination columns in the Intake/Elimination Configuration form.
